Alleged Violation Description OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards associated with inadequately maintained racking systems:  a) Carosella & Russo Enterprises located at 1600 Washington Avenue, Philadelphia PA- Warehouse: On or about August 1, 2023, employees operated a forklift to load and unload merchandise from damaged steel racking system with bent upright columns and improperly added column extensions. The racking system was used to store pallets, lumbar and aluminum studs, exposing employees working near the damaged racks to falling objects and rack collapses.
