29 CFR  1926.451(b)(1):Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    On or about August 2, 2023, on the front side of a newly built residential home, employees were exposed to a fall hazard when working from fabricated frame scaffold to perform stucco work without each level fully planked between the front uprights and guardrail supports.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about August 2, 2023, on the  front and sides of  the newly built residential home, employees were exposed to a fall hazard exceeding 10 feet when performing stucco work from fabricated frame scaffolds without a portable ladder, hook-on ladder, attachable ladder, or other safe means of access.

29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews.  On or about August 2, 2023 on the front side and each side of the newly built residential home,  two employees were exposed to a fall hazard while working from third level of a fabricated frame scaffold performing stucco work without a guardrail system in place.

On or about August 2, 2023, on the side(s) and front of the newly built residential home, employees were exposed to a fall hazard when performing stucco work from fabricated frame scaffold without all frame and panels braced by cross, horizontal, or diagonal braces, or combination thereof.
