29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not in use:    a) Job site, 1100 Washington Road, Mount Lebanon, PA: On or about August 3, 2023, during siding activities, the employer did not ensure that a safe means of getting on/off of a scaffold was in use in that an employee climbed on the triangular bracing at the top of a pump jack scaffold in order to get down from it, which exposed him to fall hazards of up to approximately 20 feet.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) Job site, 1100 Washington Road, Mount Lebanon, PA: On or about August 3, 2023, during siding activities, the employer did not ensure that a ladder, used to get on/off the scaffold, was properly set up in that it only extended about 1 1/2 feet above the roofline of the home where a fall hazard of approximately 20 feet existed.
