29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not post and maintain proper warning signs where such a circuit exists. The employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. a) Jobsite located at 148 West Cumberland Street Philadelphia, PA.: Prior to conducting siding activities from the working platform of a pump jack scaffold, approximately 25 feet above the ground level, the employer did not post warning signs or advise the employees of the location and hazards of the 13.2kv overhead power line within approximately 2 feet from the work activity, and the protective measures to be taken, thus exposing the employee to electric shock hazards. Documented to occur on or about August 8, 2023.
