29 CFR 1904.4(a):  The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  Amazon DBK1, 1 Bulova Avenue, Flushing, NY 11377: The employer did not record the following workplace injuries or illnesses on the OSHA 300 Log for the calendar year 2023:   a) On or about 8/5/2023, the employer failed to record a work-related injury that occurred on or about July 23, 2023. An employee was manually lifting packages from a conveyor belt and repeatedly bending down to place packages into Tote bags at floor level and sustained a knee injury resulting in medical treatment beyond first aid in the form of on recommended stretching beyond warm up stretching completed pre shift on or about July 29, 2023.
