29 CFR1910.23(c)(4):  Portable ladders were not used on stable level surfaces and not secured or stabilized to prevent accidental displacement.  Worksite:  On or about August 9, 2023, the employer did not ensure an employee using a 6 foot step ladder assessed and placed the ladder on stable ground prior to use, resulting in an elbow fracture and dislocation.
