29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Front and east side of house roof - On or about August 8, 2023 the employer did not ensure employees were protected from falling off the roof with the use of a fall protection system exposing employees to fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Front of house - On or about August 8, 2023, the employer had his employees use a step ladder to gain access to the roof that did not extend 3 feet past the roofs edge exposing employees to fall hazards.
