29 CFR 1910.1200(g)(8): The employer shall maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and shall ensure that they are readily accessible during each work shift to employees when they are in their work area(s). (Electronic access and other alternatives to maintaining paper copies of the safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.) On or about August 8, 2023, employees engaged in minion tank cleaning and maintenance were exposed to chemicals including but not limited to DOT 3 brake fluid corrosive hazards due to not having the safety data sheets (SDS) available in their work area.

29 CFR 1904.7(a):Basic requirement. You must consider an injury or illness to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness. You must also consider a case to meet the general recording criteria if it involves a significant injury or illness diagnosed by a physician or other licensed health care professional, even if it does not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness. On or about August 8, 2023 employer failed to record a job related injury on the OSHA 300 log injury and illness record for employee, which occurred on or about October 2023, that resulted in days away from work and medical treatment beyond first aid.

29 CFR 1910.151(c):Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. On or about August 8, 2023, employees engaged in the Minion tank cleaning and maintenance were handling corrosive such as DOT 3 brake fluid, exposing employees to eye hazards due to not having an eye wash station or eye wash bottle available at the work area.
