29 CFR 1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a. Train Station Meats & Market LLC located at 161 Edison Avenue, Circleville, OH 43113: On or about July 22, 2023 and at times prior, the employer did not develop and implement an energy control program which established energy control procedures, employee training and periodic inspections when employees were engaged in servicing/maintenance and cleaning activities on machinery which included but was not limited to the Hollymatic forming/portioning machine and the Hobart Meat-cutting Bandsaw. Employees performed set-up/break-down, periodic maintenance, cleaning and blade changes on the equipment. A hazardous energy control program was not developed and implemented to prevent unexpected energizing, startup or release of stored energy thereby exposing employees to struck-by and caught-in hazards.

Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. Train Station Meats & Market LLC located at 161 Edison Avenue, Circleville, OH 43113: On or about August 10, 2023 and at times prior, the unused portion of the blade of a Hobart Meat Cutting Bandsaw Model 5216, Serial Number: 27-020-622 was not guarded, thereby exposing employees to a struck-by hazards.

Train Station Meats & Market LLC located at 161 Edison Avenue, Circleville, OH 43113: On or about July 22, 2023 and at time prior, the employer did not ensure that a work-related incident resulting in an amputation was reported within 24 hours. An employee sustained amputation injuries to two fingers while setting-up Hollymatic Forming/Portioning Machine (Patty Machine) on July 22,2023 which was not reported to OSHA.
