Train Station Meats & Market LLC located at 161 Edison Avenue, Circleville, OH 43113: On or about July 22, 2023 and at times prior, the employer did not develop and implement an energy control program which established energy control procedures, employee training and periodic inspections when employees were engaged in servicing/maintenance and cleaning activities on machinery which included but was not limited to the Hollymatic forming/portioning machine and the Hobart Meat-cutting Bandsaw. Employees performed set-up/break-down, periodic maintenance, cleaning and blade changes on the equipment. A hazardous energy control program was not developed and implemented to prevent unexpected energizing, startup or release of stored energy thereby exposing employees to struck-by and caught-in hazards.
