29 CFR  1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.  Location - 59 Green Street, Malden, MA.  There were two instances in which employees were exposed to electrical hazard while working on a scaffold.  Instance A) On or about 8/1/2023, two employees erected and worked on a tubular welded frame scaffold within the approach distance of unprotected live electrical lines.  Instance B) On an unknown date two employees dismantled the tubular welded scaffold system that was set up within the approach distance of unprotected live electrical lines.

29 CFR  1904.32(a)(2): At the end of each calendar year, the employer did not create an annual summary of injuries and illnesses recorded on the OSHA 300 Log.  Location -   On or about 8/16/2023, the employer does not create, maintain, or post OSHA 300/300A logs annually.
