(a) Courtyard - An employee erecting a tubular welded frame scaffold was exposed to a fall hazard of approximately fifty-six (56) feet in that a hook-on or attachable ladder was not installed after the scaffold erection had progressed to a point that permitted safe installation and use, on or about August 18, 2023. Abatement Certification Required.
