Location: 2 County Manor, Ipswich MA On 8/21/2023, employees faced the risk of serious injury from fall associated with an improperly setup pump jack scaffold. The scaffold platform was installed with a cantilever of greater than 48" and access to this portion was not blocked.

Location: 2 County Manor, Ipswich MA On 8/21/2023, employees faced the risk of serious injury from falls and struck-by hazards associated with an improperly supported pump jack scaffold. Instance A: An employee faced a risk of serious injury from falls while working on the platform of a pump jack scaffold that was improperly supported. The scaffold poles had been set up without mudsills and lacked a stable foundation, causing the entire scaffold to become unstable and shift. Instance B: An employee faced a risk of serious injury from struck-by hazards while working near a pump jack scaffold that was improperly supported. Instance A: An employee was at risk of serious injury due to falls while on the platform of a pump jack scaffold lacking proper bracing. Absence of rigid triangular bracing or its equivalent at the scaffold's base led to instability, causing the entire scaffold structure to shift.
