OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees are constantly walking/working in between non-anchored pallet racks, thus exposing them to a struck by and/or crush hazard.  On or about: 8/16/2023 Location: 400 Corporate Dr., Blauvelt, NY 10913  a) Furniture racking system  was not anchored to the ground, which are approximately 15ft high. Employees are walking/working and driving forklifts between multiple racks daily. Employees are exposed to struck by and/or crushing hazard.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  On or about: August 22, 2023 Location: 400 Corporate Dr., Blauvelt, NY 10913  a) Two fire extinguishers were observed blocked by product and boxes preventing ease of access to the fire extinguishers.
