29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective in the following area(s):  On or about August 23, 2023, chemical trailer Unit 3 in front of the Derrick House stairs, the trailer was observed not having a proper path to ground, exposing employees to electrical shock hazards.

29 CFR  1910.1200(b)(4)(i): In work operations where employees only handled chemicals in sealed containers which were not opened under normal conditions of use, the employer did not ensure that labels on incoming containers of hazardous chemicals were not removed or defaced in the following area(s):  a) On or about August 23, 2023, on chemical trailer Unit 3 in front of the Derrick House stairs, a sealed chemical container containing EFOS 5132- Oxygen Scavenger was observed without a label, exposing employees to eye damage and mild skin irritation.   b) On or about August 23, 2023, on chemical trailer Unit 3 in front of the Derrick House stairs, a sealed chemical container containing EFC 6005- Corrosion Inhibitor was observed without a label,  exposing employees to respiratory, eye, and skin irritation.
