Juan Salinas DBA S Interior Drywall LLC on or about August 25, 2023 at the location of 7629 Sylvan Towne Rd. Sylvania, OH 43560: The employer failed to instruct employees in the recognition and avoidance of hazards associated with working from heights and powered industrial truck operations. Without the benefit of this training, employees were not equipped with the information necessary to identify common hazards such as falls and struck-by hazards present or likely to be present in their work environment.

29 CFR  1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet or more to lower levels by guardrail systems or personal fall arrest systems.  a). Juan Salinas DBA S Interior Drywall LLC on or about August 25, 2023 at the location of 7629 Sylvan Towne Rd. Sylvania, OH 43560: The employer did not ensure employee working from heights greater than 6' were adequately protected from falling to a lower level. Employees observed landing a crate of countertops at a hoisting location then transferring the material from a Genie GTH-844 telehandler forklift from an unguarded apartment balcony without the benefit of fall protection were exposed to a falls of approximately 19 ft to the level below.
