OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) 4725 Charles Rd. Mechanicsburg, PA 17050 - On or about August 29, 2023, employees were performing tree removal tasks from the basket of a Bil-Jax towable boom lift in excess of 10-feet above grade, and was utilizing a Shock Absorbing Lanyard that was anchored to the basket of the boom lift and to the worker's pants belt which did not constitute a complete personal fall arrest system, thereby exposing employees to a fall and catapult hazard.  b)  4725 Charles Rd. Mechanicsburg, PA 17050 (West side of residence)- On or about August 29, 2023, workers were performing tree service operations and had vehicles with a Vermeer BC1000XL Brush Chipper and trailer attached to them in which they were utilizing and accessing for work operations.  The vehicle and associated equipment/trailers were positioned in the northbound lane of the roadway of the activity area.  A traffic taper was not constructed in the transition area and signage was not posted in the advanced warning area to the south of the work operations to inform road users of what actions the road user must take to drive through the temporary traffic control zone, thereby exposing employees in the temporary traffic control zone to struck-by hazards.

Mechanicsburg, PA 17050 - On or about August 29, 2023, employees were removing cut tree limbs from ground level directly below another employee operating a chainsaw and cutting the limbs from the tree allowing them to fall to the ground, and the employer did not ensure that employees were wearing head protection, such as a hard hat.  No abatement certification or documentation required.
