29 CFR 1910.303(b)(2):Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. At the workplace located at 1500 North Congress Avenue Kansas City, MO 64153, employees were exposed to fire hazards by interconnecting multiple relocatable power strips. 1. At the security desk there were two relocatable power strips interconnected through the security fence. 2. In section D7 there were two relocatable power strips interconnected under a workstation. 3. Relocatable power strips were permanently affixed to workstations.

29 CFR 1910.178(q)(7): Maintenance of industrial trucks. Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. At the workplace located at 1500 North Congress Avenue Kansas City, MO 64153, employees were exposed to struck-by, crushed-by, and ejection hazards, in that the employer did not ensure that forklifts were being inspected for defects before use.
