29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 8/29/2023, at 1674 S Park Ave, Titusville, FL: employees and subcontractor used an aluminum ladder that only extended approximately 18 inches past the landing surface to access the 18-foot-high roof, exposing employees to fall hazards. b) On or about 8/31/2023, at 1674 S Park Ave, Titusville, FL: subcontractor employees used an aluminum extension ladder that only extended approximately 28 inches past the landing surface to access the 18-foot-high roof, exposing employees to fall hazards.
