Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall, struck by, and caught between hazards associated with the installation of prefabricated parallel chord trusses without any temporary bracing: Site- Employees working from and beneath prefabricated parallel chord floor trusses were exposed to fall, struck by, and caught between hazards where temporary or permanent bracing was not installed during the course of installation.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: Site- An employee working from the top chord of prefabricated parallel chord floor trusses was not protected from free falling more than 6 feet and contacting the floor below where the anchor point for their personal fall arrest system was located more than 6 feet away from them horizontally, at their feet.
