(a) In the maintenance shop at 122 Mill Road, McDonough, Georgia: On or about September 5, 2023, and at times prior, employees were exposed to a fall hazard of more than 13-ft. in that the employer did not place "Dangerous: Do Not Use" tags on or remove from service, the two 12-step rolling metal ladders, that were missing their brake and locking mechanisms.

Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.  (a) In the maintenance shop at 122 Mill Road, McDonough, Georgia: On or about September 5, 2023, and at times prior, employees were exposed to a fall hazard of more than 13-ft. in that the employer modified the capacity and safe operation of the forklift without prior written approval of the manufacture. The employer constructed a basket to lift employees up to 13-ft. utilizing the forklift.
