A fabricated frame scaffold was erected without a competent person, thereby exposing employees to struck-by and up to 30 feet fall hazards.

Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  a. American Continent Construction LLC., at a worksite located 2335 North Cassidy Avenue, building 4, Columbus, Ohio 43219:  On and preceding September 6, 2023, the employer did not ensure each employee working from scaffolding was protected from falls. Employees were performing masonry work from a fabricated frame scaffold without any form of fall protection in place, thereby being exposed to a 30-foot fall hazard.
