29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) At the worksite, On or about 9/07/2023, employees were working on a low sloped roof approximately 18-feet above the lower level without any means of fall protection while making repairs to a vent pipe.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) At the worksite: On or about 9/07/2023, employees were utilizing a portable extension ladder to access a roof approximately 18-feet above the lower level, that did not extend at least 3 feet above the upper landing surface.
