29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  (a)	On or about September 7, 2023, the employer did not ensure employees were provided with hard hat while working in the Silo 2 unit where a roll sleeve was jammed inside at the top of the elevator. The employee was exposed to potential falling objects from above.

29 CFR 1910.147 (d)(4)(i):  Lockout or tagout devices was not affixed to each energy isolating device by authorized employees:  (a)	On or about September 7, 2023, employees working in Pre-Press Department unjamming the Silo 2, model:  S2-3EE40AN, serial:  202045 where a lock and tag was not applied. The employee was exposed to electrical hazards, gravitational and/or mechanical hazards and the hazard of unexpected energization which would expose the employee to an amputation.
