On or about 9/5/23, on the roof.  Workers were installing roofing materials onto a 3:12 pitch roof with a ground to eave height of up to 10 feet without fall protection of any kind.

Abatement certification is required for this item.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 1233 Erie Boulevard West, Syracuse, NY  13204.  On or about 9/5/23, on the roof.  Workers were accessing a roof surface at 10 feet from ground with a portable ladder that did not extend at least 3 feet above that landing.
