29 CFR  1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  On or about Sept 15, 2023, and at times prior thereto, the subcontractor did not ensure employees working from heights six feet or greater were provided with fall protection. The sub-contractor and his employee were both exposed to a fall hazard of approximately twelve feet to the concrete below.

29 CFR  1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  On or about Sept 15, 2023, and at times prior thereto, a-frame ladders that were not tall enough to access and egress the work area, employees used the top steps to jump up onto the upper landing surface exposing them to fall hazards approximately 12 feet to the concrete below.
