29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  43 Rhinebeck Ave, Springfield, MA: On or about September 20, 2023, two employees using a JLG 600SJ aerial lift, climbed out of the lift basket to access a roof. One employee was observed sitting on the edge of the basket and the other was standing directly on the roof while performing roof tile removal.

29 CFR  1926.502(d)(6)(i): A snaphook not of the locking type was engaged directly to webbing, rope, or wire rope such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.   43 Rhinebeck Ave, Springfield, MA: On or about September 20, 2023, two employees working from a JLG aerial lift were observed with improperly configured personal fall arrest systems. The metal snaphook was wrapped around the frame of a JLG aerial lift and attached back to the vertical lifeline while employees performed roofing activities.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  43 Rhinebeck Ave, Springfield, MA: On or about September 20, 2023, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for two employees who were performing construction work and exposed to fall hazards of approximately 24 feet.
