29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   170 Country Rd., Southampton, MA:  On or about September 21, 2023, an employee was observed using a portable aluminum extension ladder to access his roof top work location that did not extend the required 3 feet past the upper landing surface.
