29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable.  a) On or about 09/22/2023; throughout the establishment, an energy control program was not established that consists of specific energy control procedures, authorized employee training, and periodic inspections to ensure employees perform energy control procedures before servicing or doing maintenance such as but not limited to, changing blades or greasing the Tire Buffing Machine. 29 CFR  1910.176(b): Material stored in tiers was not stacked, blocked, interlocked, or limited in height so that it was stable and secure against sliding and collapse:  a) On or about 09/21/2023; in the shop, employees were exposed to struck-by hazards as tires, weighing approximately 75 lbs., were stored on racks at a height of around 15 feet without securing them against sliding.  An employee was fatally injured when a tire that was stored vertically on its tread, on top of a small rim, was accidentally hit by the movement of a ladder, causing the tire to roll off the rack and strike the employee.  ABATEMENT DOCUMENTATION REQUIRED
