A. Jobsite - garage: The folded 6-foot step ladder, used by employees accessing/egressing between the garage and 1st floor, was not secured and did not extend 3-feet beyond the 1st floor.  B. Jobsite - Basement stair opening: The siderails of the 9-foot fly section of the aluminum extension ladder, used by employees accessing/egressing the basement, was not secured and did not extend 3-feet above the 1st floor.
