29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about September 25, 2023, on the north side of a residential home, employees working on a roof without having been trained in the following instances:   a)  Employees were installing asphalt shingles on the existing roof; b)  Employees were working at the unguarded edge of the roof; c)  Employees were not protected from fall hazards by using personal fall arrest or fall prevention equipment.

29 CFR 1926.1053(b)(22):  Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:  On or about September 25, 2023, on the north side of a residential home, employees were climbing a Werner extension ladder while carrying roofing material.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about September 25, 2023, on the north side of a residential home, employees engaged in roofing work did not have any fall protection systems in place to protect them from falling greater than 6-feet to the ground below.  Acosta Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1559234, Citation 1, Item 2 and was affirmed as a final order on April 1, 2022, with respect to a workplace located at 110 Bouries Circle, Maumelle, AR 72120.
