29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about September 22, 2023 and at times prior thereto, employees perform maintenance work on vehicles and the employer did not develop, document, and utilize procedures to control all sources of energy.  This exposed the employees to struck-by and caught-in hazards.

29 CFR  1910.147(d)(5)(i):  All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices:  On or about September 22, 2023, an employee performed maintenance on a vehicle and did not ensure all hazardous stored or residual energy was restrained and rendered safe before beginning work on the vehicle.  This exposed the employee to struck-by and caught-in hazards.

An in-patient hospitalization affecting one (1) employee occurring on or about September 22, 2023, was not reported to the local OSHA area office within the 24-hour reporting requirement.
