29 CFR  1910.147(d):  The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about December 20, 2023, at a facility located at 246 Trowbridge Drive in Fond du Lac, WI; employees were exposed to machine hazards associated with moving parts when they engaged in sanitation procedures:  b)  The employer did not ensure that employees implemented energy control application steps or equipment protective measures prior to allowing them to clean and sanitize the Line 4 Scale Infeed Conveyor that fed the Line 4 scales above the RapidPak machine. Sanitation employees used water hoses and their hands to wash the energized and operating conveyor belt in the areas immediately adjacent to the tail pulley's ingoing nip point and the head pulley's ingoing nip point. The machine was not deenergized prior to the employees engaging in the servicing and maintenance activities.  c) The employer did not ensure that employees implemented verification of isolation and deenergization prior to allowing them to clean and sanitize the Line 4 Z-belts machine. A Sanitation employee isolated and locked out the Z-belt machine; however, the employee did not attempt to start the machine after applying his lock and before conducting sanitation work. The machine was not verified to be isolated and deenergized prior to employees engaging in the servicing and maintenance activities.  As a result, the applicable energy control elements, involving preparation for shutdown [1910.147(d)(1)], machine shutdown [1910.147(d)(2)], machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  On or about December 14, 2023, at a facility located at 246 Trowbridge Drive in Fond du Lac, WI; machine guarding was not provided to protect operators and other employees in the machine areas from hazards such as, but not limited to:  a)  On Dice/Shred packaging lines 1, 3, and 4; ingoing nip points presented by the clear rubberized conveyors located immediately downstream of the check weigher equipment, which exposed employees to amputation hazards where the conveyors ran over powered pullies.  b)  On the Slice Line at the Multivac; ingoing nip points presented by the blue conveyor immediate downstream of the end of the Multivac machine, which exposed employees to amputation hazards where the conveyor ran over the head pulley at the side of the belt and via a hole in the center of the belt.  c)  On the Triangle line; ingoing nip points presented by the blue rubberized conveyor on the underside of the conveyor's tail pulley, at the underside of the lower return bend pulley, and at the underside of the carry-side top bend pulley above the scale platform, which exposed employees to amputation hazards.
