29 CFR  1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish 1928.57(a)(11)(i-iii):   a) On or about 9/28/2023 at the Barn, by Pen 14 and Alley Scraper Drive Unit,  AS21, Will-O-Crest Farms LLC, 800 County Road 27, Clifton Springs, New York 14432, an employee was exposed to a slip hazard while inspecting the cable on the drum of the Alley Scraper Drive Unit, AS21,  while it was operating and without its cover.   The employee slipped on the floor and as the employee fell, the employee's right arm became entrapped between the Alley Scraper Drive Unit's rotating drum and side wall, resulting in multiple fractures to the employee's right arm.  Abatement certification is required for this item.
