29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a. On or about October 2, 2023, an employee performing siding work at a commercial property was exposed to a fall hazard of 8'-8"with no fall protection. The employer failed to ensure the employee was adequately trained to recognize and minimize the hazards associated with fall.

On or about October 2, 2023, an employee was using an extension ladder to gain access and egress from the roof area. The ladder was extended more than the 4:1, thereby exposing the employee to fall hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  On or about October 2, 2023, at the above addressed jobsite, an employee performing commercial siding activities, eight (8) feet eight (8) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  Carlos Delgado dba Carlos Delgado was previously cited for a violation of its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1428657, citation number 1, item number 1 and was affirmed as a final order on November 14, 2019, with respect to a workplace located at 2911 Alden Ave. Yorkville, Illinois 60560.
