29 CFR 1926.25(a): Housekeeping. During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. On or about October 2, 2023, at 2369 Hidden Winds Lane in Green Bay, Wisconsin, an employee was exposed to trip hazards walking and working in an area without a clear pathway. Trusses and construction debris covered the walking surface.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) On or about October 2, 2023, at 2369 Hidden Winds Lane in Green Bay, Wisconsin, an employee was exposed to fall from elevation hazard while working from an exterior wall top plate 10 feet above surface level without using fall protection equipment. b) On or about October 2, 2023, at 2369 Hidden Winds Lane in Green Bay, Wisconsin, employees were exposed to fall from elevation hazards while working from interior wall top plates at 9 feet above the wooden subfloor surface without using fall protection equipment.

29 CFR 1926.503(b)(1): Training Requirements. Certification of training. The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. On or about October 2, 2023, at 2369 Hidden Winds Lane in Green Bay, Wisconsin, the employer did not provide written fall protection safety training certifications for employees working at heights greater than six feet while setting residential trusses.
