29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) 4869 S Hoyt St, Littleton, CO 80123: On or about October 07, 2023 and at times prior, TSV Construction and Roofing Lone Tree as a controlling employer, did not ensure that subcontractor employees conducting roofing operations were safe from health and safety hazards by conducting effective frequent and regular safety inspections of the jobsite by a competent person, and ensuring hazards, such as but not limited to fall hazards were identified and mitigated. Sub contractor employees were exposed to fall hazard by not being tied off while performing roofing operations.

The employees did not have fall protection onsite, and fled the jobsite when CSHO tried to speak with them.
