29 CFR 1926.453(b)(2)(v): A body belt or fall protection harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) 213 North Front Street, Harrisburg, PA; at the front part of the roof - On or about October 3, 2023, the employer did not ensure that the employee operating the Genie Z-62/40 was wearing a fall protection harness and lanyard while working at 30 feet above the lower level. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) 213 North Front Street, Harrisburg, PA; at the front part of the roof - On or about October 3, 2023, the employer did not ensure that the employee operating the Genie Z-62/40 at 30 feet above the lower level was trained to recognize fall hazards and procedures to minimize those hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
