29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  At a commercial worksite located at 7113 N Troost Ave, Gladstone, MO 64118, employees were not wearing eye protection while tearing off old shingles. This exposed employees to getting debris in their eyes causing partial / temporary blindness. This was documented on or about 10/3/2023.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.   At a commercial worksite located at 7113 N Troost Ave, Gladstone, MO -- employees engaged in roofing duties on a low-slope roof were not using any kind of fall protection system. This exposed employees to fall hazards greater than 19 feet to ground level. This was documented on or about 10/3/2023.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a commercial worksite located at 7113 N Troost Ave, Gladstone, MO -- employees engaged in roofing duties were using an extension ladder that was not extended 3 feet above the landing surface -- the roof being the landing surface in this case. This exposed employees to fall hazards of 19 plus feet to ground level. This was documented on or about 10/3/2023.
