The left side of the stairs were not equipped with railings. The stairs have 9 risers and is roughly 6 feet off the ground.

Equipment was used with tape not allowing employees to properly inspected the rigging equipment.  B) American Hard Chrome LLC., on or about October 04, 2023, at the address of 925 Industrial Street, New Castle, PA: Employees did not inspect slings and fastening attachments prior to use each day. Equipment was used with the outer protective cover of the slings being damaged or torn.
