29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c).  (a) 310 E Railroad Ave N, (front of building) - On or about October 5, 2023, the employer instructed an employee to weld  in a 5 ft. trench and he did not ensure that the trench walls were shored, adequately benched or otherwise protected, exposing employees to a cave-in hazard.
