29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not appropriately worn by employee(s) working from an aerial lift.  On or about October 4, 2023, the employer did not sure that employees engaged in construction activities 6 feet above the lower level were wearing fall protection. This condition exposed the employees to fall hazards.

29 CFR  1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.   On or about October 4, 2023, employees were exposed to fall hazards in excess of 6' while engaged in commercial construction activities were not trained for fall hazards on aerial lifts.
