29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about October 3, 2023, at jobsite located at 462 Simms St., Aurora IL 60505, employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun. Eye protection was not being utilized while employees were engaged in this activity. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

Fall protection was not being utilized while performing roofing activities.
