29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about October 3, 2023, at jobsite located at 462 Simms St., Aurora IL 60505, employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun.  Eye protection was not being utilized while employees were engaged in this activity.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  On or about October 3, 2023, at jobsite located in 462 Simms St., Aurora, IL 60505, employees were exposed to fall hazards in excess of 15 feet while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities.
