(a) Residential roof structure, various locations; The employer did not prepare a written certification record to verify training was provided on applicable topics, including but not limited to fall hazard recognition and appropriate mitigation methods, for employees that were exposed to fall hazards ranging between 9 feet to 16 feet, 2 inches above ground level.
