29 CFR 1910.178(l)(4): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. At 3433 Northwest 18th Street, Topeka, Kansas, 66618, on the production floor, forklift operators were not provided with forklift refresher training every three years to operate the YALE forklift, exposing all employees working in this area to a potential struck by hazard.
