29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 10/13/2023, at a home and garage roof project located at 5825 McKinley Pkwy in Hamburg NY; employees were installing shingles on the roof of a two-story home. The roof had multiple pitches including a 5 in 12 pitch with an eave height of approximately 20 feet.  There was no fall protection in use.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) On or about 10/13/2023 at the roof area of the site, side rails of the extension ladder did not extend 3 feet above the upper landing surface being accessed. Employees were exposed to falls.   NO ABATEMENT CERTIFICATION REQUIRED
