The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  At a facility located at 7811 County Road D in Waupaca, Wisconsin, the employer did not ensure that employees implemented energy control application steps or equipment protective measure prior to allowing them to perform work on equipment such as the following:  a) On October 18, 2023, an employee was observed working on the shanklin machine and did not utilize a lockout/tagout device.  b) On October 18, 2023, an employee was observed working on the mixer and did not utilize a lockout/tagout device.  c)   On October 18, 2023, the employer did not protect the employees from hazards associated with moving parts while entering a spiral freezer while the freezer is operating and not performing lockout/tagout.   As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.

Lower guard on the pie press to ? inch opening above pie pan, no later than April 12, 2024.   b) On or about November 1, 2023, the employer did not protect employees from the ingoing nip points on the dough roller on line 1. Add a second bar to the dough roller to increase distance from feed, no later than April 12, 2024.  c) On or about October 18, 2023, the employer did not protect employees from the nip point on the tex-wrap machine. Add plexiglass tunnels to the tex-wrap machine entry and exit, no later than April 12, 2024.
