29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Baldor Tool Grinder / Polisher #1: On or about October 18, 2023, sufficient point of operation guarding over the abrasive belt grinding head was not provided to protect operator from ingoing nip points and rotating drive wheel. (b) Baldor Tool Grinder / Polisher #2: On or about October 18, 2023, sufficient point of operation guarding over the abrasive belt grinding head was not provided to protect operator from ingoing nip points and rotating drive wheel. (c) Baldor Tool Grinder / Polisher #3: On or about October 18, 2023, sufficient point of operation guarding over the abrasive belt grinding head was not provided to protect operator from ingoing nip points and rotating drive wheel. (d) Baldor Tool Grinder / Polisher #4: On or about October 18, 2023, sufficient point of operation guarding over the abrasive belt grinding head was not provided to protect operator from ingoing nip points and rotating drive wheel. (e) Baldor Tool Grinder / Polisher #5: On or about October 18, 2023, sufficient point of operation guarding over the abrasive belt grinding head was not provided to protect operator from ingoing nip points and rotating drive wheel. (f) Pallet Line: On or about October 18, 2023, employees working at or around the pallet line were exposed to caught-in hazard and ingoing nip point in-that the three sections of barrier guarding mounted over the in floor sand conveyor was not installed and secured in a manner that fully covered the operating belt conveyor.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end, and projected more than one-half the diameter of the shaft without being guarded by nonrotating caps or safety sleeves: (a) On or about October 18, 2023, at the Baldor Grinder / Polisher #1 an exposed two (2) Inch diameter rotating shaft end located approximately 32 inches above the floor, projected approximately two (2) inches past the grinding belt drive wheel was not adequately guarded to prevent employee access. (b) On or about October 18, 2023, at the Baldor Grinder / Polisher #2, an exposed two (2) Inch diameter rotating shaft end located approximately 32 inches above the floor, projected approximately two (2) inches past the grinding belt drive wheel was not adequately guarded to prevent employee access. (c) On or about October 18, 2023, at the Baldor Grinder / Polisher #3, an exposed two (2) Inch diameter rotating shaft end located approximately 32 inches above the floor, projected approximately two (2) inches past the grinding belt drive wheel was not adequately guarded to prevent employee access. (d) On or about October 18, 2023, at the Baldor Grinder / Polisher #4, an exposed two (2) Inch diameter rotating shaft end located approximately 32 inches above the floor, projected approximately two (2) inches past the grinding belt drive wheel was not adequately guarded to prevent employee access. (e) On or about October 18, 2023, at the Baldor Grinder / Polisher #5, an exposed two (2) Inch diameter rotating shaft end located approximately 32 inches above the floor, projected approximately two (2) inches past the grinding belt drive wheel was not adequately guarded to prevent employee access.
