29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated: (a) Jobsite at 31 S. Grove Avenue, Elgin, IL 60120 - On or about October 18, 2023, an employee performing caulking operations from the platform of a JLG 460SJ telescopic boom lift, around 21 (twenty-one) feet above the ground, was wearing a personal fall arrest harness without straps secured buckled around legs, thereby exposing the employee to fall hazards. Abatement documentation is not required of this item.

29 CFR 1926.503(b)(1): The employer did not verify fall protection training by preparing a written certification record. (a) 31 S. Grove Avenue, Elgin, IL - On or about October 18, 2023, the employer did not certify that employees had received fall protection training prior to requiring them to work at a height of 21 (twenty-one) feet from a JLG 460SJ telescopic boom lift. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
