29 CFR 1910.29(a)(2): The employer did not provide and install all fall protection systems and falling object protection this subpart requires, and comply with the other requirements in this subpart before any employee begins work that necessitates fall or falling object protection:  a) Jobsite located at 2200 Walnut St, Philadelphia, PA: On or about October 19, 2023, the employer did not provide and install all fall protection systems and falling object protection this subpart requires, and comply with the other requirements in this subpart before any employee begins work that necessitates fall or falling object protection. Employees were performing elevator modernization work from the top of an elevator cab parked at the 6th floor without the use of fall protection, thereby exposing employees to fall hazards of approximately 60 feet down the elevator shaft.

29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:   a) Jobsite located at 2200 Walnut St, Philadelphia, PA: On or about October 19, 2023, the employer did not ensure sprocket wheels and chains which were seven (7) feet or less above floors or platforms were enclosed. Three employees were working on elevator modernization activities in the basement elevator machine room in proximity to unguarded sprocket and chains thereby exposing employees to caught-in hazards.
