Employees were performing elevator modernization work from the top of an elevator cab parked at the 6th floor without the use of fall protection, thereby exposing employees to fall hazards of approximately 60 feet down the elevator shaft.

Employees were performing elevator modernization work from the top of an elevator cab parked at the 6th floor. The cab was secured with alloy steel chains equipped with hooks with damaged safety latches, thereby exposing employees to fall hazards of approximately 60 feet down the elevator shaft.
