On the roof of a temporary residential worksite, located at 3932 DuPont Circle, Jacksonville, FL 32205: On or about October 18, 2023, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection were not utilized by the employees, while using nail guns to install roof shingles.

On a 5:12 pitched roof of a residential worksite,  located at 3932 DuPont Circle, Jacksonville, FL 32205:  On or about October 18, 2023, the employer exposed 5 employees to fall hazards ranging from 12 to 20 feet, in that, fall protection was not utilized by the employees while performing roofing activities.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   a. On a temporary residential job site located at 3932 DuPont Circle, Jacksonville, FL 32205, On or about October 18, 2023, the employer exposed five employees to a 12-foot fall hazard, in that the ladder's side rails did were only extended 1.5 rungs above the roof and was not tied off to prevent displacement.
